Reiki

I understand that Reiki is a simple, gentle, hands-on energy technique that is used for stress reduction and relaxation.  I understand that Reiki practitioners do not diagnose conditions, nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. I understand that Reiki does not take the place of medical care. It is recommended that I see a licensed physician or a licensed healthcare professional for any physical or psychological ailments I may have period I understand that Reiki can compliment any medical or psychological care I may be receiving. I also understand that the body can heal itself and to do so, complete relaxation is often beneficial, I acknowledge that long term imbalances in the body sometimes require multiplesessions in order to facilitate the level of relaxation needed by the body to heal itself.

Hypnosis

The undersigned that he/she is at least 18 years of age and that he/she understands that good and lasting results may require several hypnosis sessions, and that he/she may be required to practice self- hypnosis and/or listen to a reinforcement recording between sessions. That he/she is responsible for cooperating with, and participating in his/her program.  The certified hypnotist shall not be responsible or held accountable for the results he/she attains from the hypnosis sessions.  He/She understands that he/she may be referred elsewhere for proper treatment, and that his/her program may be terminated if deemed appropriate.  When listening to hypnosis recordings, do so in a safe place, preferably where you will not be disturbed for the duration of your recording.  Never listen to recordings or practice self-hypnosis while driving in a car, operating machinery, or doing anything else that requires your attention for safety reasons.
